Does the lid fall off when pouring?

Not at all. It’s locked in with plastic and has two pouring options. Direct open stream or a sieve to strain if you add fruit to your pitcher.

If dishwasher safe, why is hand washing recommended?

My wife cleans our pitcher in the dishwasher. No problems so far. They may recommended hand washing because of the shape.

Is the glass thick or thin? Meaning is it easily breakable?

Very sturdy not so thick that it is awkward to hold. Good weight to the carafe

Can I use this on my gas cooktop to heat water?

Yes, This is a heat resistant glass pitcher, you could use it on gas and stove.
